I, the applicant, and my personal representatives, assigned successors, heirs, and executors, do hereby release, indemnify, waive, hold harmless and forever discharge, Family Friend, Inc. and A Talent Showcase Productions: as well as its agents, officers, employees, members, and volunteers for any claims, demands, causes of action, suits, judgments (including costs and expenses) and any and all liability arising directly or indirectly from damages of all kinds, theft, costs, property loss, bodily injury, or death that I might sustain as a result of my participation in or otherwise arising out of my activity at The 2016 Juneteenth Gospel Fest.

I have read all the Event Rules & Regulations and agree to abide by them, and that my refusal to do so may result in ejection from the event or further action.

I am solely and legally responsible for my equipment, people, and product.

before applying. Upon receipt and approval of your application you will receive a confirmation email. Once you are approved, your e-form will be considered as valid as a paper contract. E-form submissions will be required to sign a Hold Harmless Waiver, in person, prior to setting up the day of the event. 

Once your application has been approved, you will receive and email with our address, and payment instructions. Please do not pay until instructed to do so.
